MO SPORTS UPDATE

25-26 PHYSICAL DATES ANNOUNCED

WED MAY 28 3-5:30PM   MO ATHLETES   MISSION OAK

TJUHSD has partnered with Adventist Health to offer FREE athletic physicals for all incoming 

9th graders as well as current high school students. 

 

Items required to be cleared to participate in athletics:

1. Online 2025-2026 Athletic Physical Clearance Registration on 

https://hello.students.arbitersports.com/    COMPLETE PRIOR TO ATTENDING

2. Complete and bring the Adventist Health Consent form signed by parent/guardian

3. Complete and bring the Pre-Medical History form signed by parent/guardian

4. Complete and bring the TJUHSD Athletic and Activity/Club Registration Form

5. Please bring a copy of your insurance card for physical clearance

 

ALL FORMS CAN BE PICKED UP AT YOUR SCHOOL NURSE’S OFFICE. 

PARENTS ARE ENCOURAGED TO ATTEND.
